Service Terms & Conditions

These Terms & Conditions apply to all À La Carte service offerings and, where applicable, to Service Plan engagements. Some terms—such as billing, travel, and overtime—may differ under Service Plans. Please refer to your Service Plan Agreement for terms specific to your selected coverage. This statement was last updated on May 12, 2025.

Validity of Rates

Rates provided are valid for 30 days from the date of delivery. Precision MammoCare, LLC reserves the right to update rates after this period, with any changes communicated in advance.

Service Area

Travel fees are based on your facility's location. Additional charges may apply for sites outside the Zone 1 Service Area. Use our Price Menu Map to check your rate.

Travel Fees

Travel fees for À La Carte service are assessed per site visit, not per service call. The travel fee for the first site visit is billed at the standard rate, with a discounted rate applied for additional site visits on subsequent days. For any site visit that incurs overtime labor charges, the associated travel will also be billed at the applicable overtime rate.

Please refer to your Service Plan Agreement for travel fee terms specific to your plan.

Additional Fees

Charges for non-standard requests (e.g., expedited scheduling) may apply. Any additional fees will be communicated prior to service.

Service Hours

  • Preventative Maintenance (PM): Regular service hours for PMs are 8am – 10pm on non- holiday weekdays*.
     
  • Repair Services: Regular service hours for repairs are 8am – 5pm on non-holiday weekdays*.

* Non-holiday weekdays refer to standard U.S. federal holidays. Additional holidays or office
closures specific to Precision MammoCare, LLC will be communicated in advance.

Overtime Policy

  • Preventative Maintenance (PM): PM services are offered at a flat rate within extended service hours (8am-10pm) on non-holiday weekdays. Should any PM service extend beyond 10pm, hourly overtime will be billed in addition to the flat rate at the “After-After Hours” rate.
     
  • Repair Services:
    • Weekday labor outside regular hours: billed at 1.5× the standard rate
    • Weekends/holidays: billed at 1.5× the standard rate on the first day; 2× for each consecutive additional day
       
  • Travel: Travel associated with any service that incurs overtime charges will also be billed at a matching overtime rate of 1.5x or 2x.

Minimum Billing Policy

Each site visit is subject to a two-hour minimum billing policy. Any additional time beyond the two-hour minimum is billed in 30-minute increments at the applicable hourly rate.

Note: This policy applies to À La Carte services only. Service Plans are billed at a flat annual rate per system.

À La Carte Payment Terms

  • Payment for À La Carte services is due within 30 days of the invoice date (Net 30). A 3% weekly interest charge (accruing at 0.429% daily) will be applied to any unpaid balance beginning 40 days after the invoice date.

    Note: Service Plans are governed by separate payment terms, as outlined in the applicable agreement.
     
  • Payments can be made via ACH or check. Please note that ACH payments made directly through any bank transfer option in your invoice will incur a 1% surcharge. For your convenience, you may also set up direct deposit payments through your banking institution. Please contact us to request our direct deposit details.

Insurance and Liability

Precision MammoCare, LLC maintains comprehensive insurance coverage for service delivery. A Certificate of Insurance (COI), outlining coverage and limits, is available upon request.
